They have to write the following under the statement • their name • their position (from the list of acceptable people) • their telephone number • the date • their signature Do not signor date the photos yourself. The digital scan, digital photo or photocopy of the photo ID and statement must be legible. Your application will be delayed if the document and statement are not clear. 2 Declaration of Non Conviction You must provide a declaration of Non Conviction for each person involved with the Authorisation this includes all company directors. A person will be considered acceptable if they do not have any unspent convictions, as defined in the Rehabilitation of Offenders Act 1974, for criminal offences connected with the MOT Service or the motor trade, or involving acts of violence or intimidation. A criminal conviction is a judgement made by a court that a person or company is guilty of any offence. The penalty imposed may range from an absolute discharge through to imprisonment. This does not include fixed penalty offences. You must inform DVSA, in writing or by email, immediately of any relevant convictions that occur between the date of application and authorisation of your application, or after authorisation.
